Service Overview

Long Term Care is intended for residents who benefit from continuous support and supervision. Care is delivered in accordance with each resident’s assessed needs and is reviewed regularly in consultation with the resident and their families/whānau.

24-hour supervision and assistance
Support with daily living, mobility, and comfort as appropriate.
Personal care support
Assistance with hygiene, dressing, continence support, and grooming.
Medication and routine monitoring
Administered and documented in line with policy and clinical direction.
Coordination with health professionals
Communication with GPs and allied health providers as needed.

Who This Service Is For
